This is to acknowledge email below. For the completion of the required documents under Medical Case Policy, kindly submit any of the following:
1. Medical Certificate and PRC ID or Doctor’s ID of the attending physician
2. Medical Certification written on Doctor’s Pad with License number of Doctor indicated
3. Medical Certification written on Hospital’s prescription pad with License number of Doctor indicated.
Please take note that the Medical Certificate should indicate the diagnosis and recommendation of the Doctor to the patient.
Please be advise also that under Medical Case policy we may only extend it to the following: one (1) companion, Immediate Family (parents, spouse, siblings, children, grandparents & parents in law) , Minors (less than15yrs) and Senior Citizens. Kindly provide supporting documents that will prove the affiliation of the other accompanying passengers to the meda guest (e.g Marriage Certificate, Birth Certificate).
Once complete documents are received, case will be subject for evaluation and approval.
