标题: [求助] 关于菲鹰航空退票提供信用卡授权书
菲鹰航空退票超级麻烦,需要提供以下:

1.Ticket

2. Facsimile of the Credit Card

3. Facsimile of the valid ID of the card owner

4. Valid ID of the duly authorized representative

5. Written authorization from the card owner that must be duly signed and acknowledged by the authorized representative concerned.

特别是最后2个项目,要怎么填写?

其实授权书很简单，只要说明信用卡持有者授权给某某处理所有机票事务即可，我英语不好，就随便写了这么一份，菲鹰工作人员表示无碍。当然英语好的话可以写得规范些。

AUTHORIZATION LETTER

Ms. XXX, the credit card holder (card no. XXXXXXX, passport no.xxx), authorize Mr.XXX (passport no. XXXX) to deal with all issues related to the ticket.

The attachment is a credit card copy.

It is hereby authorized.

Authorizer signature: _____________________

Attorney signature: _______________________

Date: 13, JAN, 2013
