关于宿雾改名费

This is to acknowledge the receipt of email below. However, underName Change Policy, the passenger is required to submit of the followingrequirements to complete such request.

           - 2 valid IDs of the original passenger  

- 2 validIDs of the new passenger

-Pro-forma Letter of Authorization (please see attached file)

Reminders:

- NAMECHANGE should be done more than 24hrs prior scheduled flight.

- PrimaryIDs: Passport, NBI Clearance, Driver’s License, SSS ID, GSIS ID, PRC ID andSenior Citizen’s ID

- SecondaryIDs: Employment ID, School ID and other private organization-issued photoIDS

- Name changeis not applicable for bookings with promo fare.

- NAMECHANGE FEE

Upon receipt of the said document, the case will be subject forevaluation and approval.

收到如上东西后才会给改名；

Name change fee for international flight is 3,000php perpassenger/per booking. Once required documents are received, case will be subject forapproval.
