小孩名字写错需要修改，宿雾回复的邮件帮忙解答下

Good day!

This is to acknowledge the receipt of the documents for name correction; however name correction policy the passenger is also required to submit of the following:

Ø       1 valid primary ID of the parent who will sign the proforma affidavit with visible signature

Ø       Proforma affidavit (attached) )– filled up by the parent

Upon receipt of the said document, the case will be subject for evaluation and approval. Please send us the necessary documents more than 4 hours prior the departure.

是网上那个签名格式书吗？要不要提供小孩出生证和我们结婚证
